Skilled Nursing

Our skilled nursing staff works to provide the highest level of care and services to meet your loved ones needs.

The CVPH Skilled Nursing Facility in Plattsburgh is home just more than 50 residents and patients of our subacute rehab program.  Our team of dedicated and compassionate professionals provides the highest quality of care and innovative programs that meet residents' health care, psycho-social and spiritual needs. 

While located within CVPH, the Skilled Nursing Facility is a separate entity but benefits from all the services provided by the hospital.  Accommodations include private and semi- private rooms, two dining rooms, two activity rooms and a laundry.

We take a team approach to caring for our residents, using the collective expertise of a number of professionals.  Registered nurses, certified nurse aides, physicians, physical therapists, occupational therapists, pharmacists, dieticians, social workers and others work as a team to be sure our residents get the very highest level of care.

  • Picnics & celebrations
  • Fishing

Residents are encouraged to eat in the communal dining room, participate in activities and dress in their personal clothing. However, individual preferences are honored. 

 

The Residents Council

Residents can take an active role in decision-making , voice grievances and resolve differences by participating in the Resident Council.  The council takes an active role in selecting activities and the quality of life within the nursing facility. Participation is voluntary but greatly appreciated.

Sub Acute

Physicians, nurses, physical, occupational and speech therapists, pharmacists and case managers work as a team to develop and implement a care plan for patients who may not require hospital care but need skilled nursing and rehabilitative care that can’t be supplied at home, in an outpatient facility or in a nursing home. 

The Admission Process

Our social worker and case manager work together to facilitate admissions to the Skilled Nursing Facility.  Completing an application is the first step.  Once complete, it can be returned to:

Discharge planning begins on the day of admission. Upon admission, an individualized care plan is developed by our team of professionals.  That plan will focus on restoring independence and assisting residents in improving their quality of life.  It is reviewed regularly and adapted as necessary.
